Athletes playing sports need to be cleared in the athletic office in order to try out or practice. The following is the clearance process for athletics:
1. Physical Exam and Acknowledgement Form ( required each year)
2. Emergency Information Form (required each year)
3. Copy of Athlete's Birth Certificate (required only once in career)
4. Athletic Informed Consent Video viewed by athlete and Parent (required each year)
5. Sportsmanship Code of conduct (required each year)
6. Brainbook Concussion Course - must be done once during your High School Career

2. Select Concussion - Brainbook picture
3. Register as a Student
4. Enter Demographic information -
5. Select sport this season and any season in the future
6. Complete course
